Asst. Prof. Dr. Mert Ersan — Facial fat grafting (autologous fat transfer) is a safe and natural technique used to restore facial volume, redefine contours, and soften signs of aging.
However, achieving long-lasting results depends not only on surgical precision but also on how well the recovery process is managed.

This article is for informational purposes only. Individualized care and follow-up should be determined by your surgeon.

The First 48 Hours: Rest and Cold Compress

The initial 48 hours after the procedure are crucial for grafted fat cells to integrate with the surrounding tissue. During this period:

  • Keep your head slightly elevated while resting to minimize swelling.
  • Apply cold compresses intermittently during the first 24–48 hours to reduce bruising and inflammation.
  • Avoid hot showers, saunas, or vigorous physical activity during the first day.

Swelling and Bruising

Temporary swelling or mild asymmetry is common and usually peaks within the first few days, gradually subsiding over 7–10 days.

  • Cold compresses are helpful for the first two days, followed by mild warm compresses from day three onward if approved by your surgeon.
  • Avoid blood-thinning medications or supplements (e.g., aspirin, fish oil, ginkgo) unless prescribed.
  • Refrain from smoking and alcohol, as they can impair circulation and reduce fat cell survival.

Massage and Touch

Facial massage is generally not recommended in the early phase following fat transfer. The grafted fat needs stability to adhere and integrate.

  • Gentle massage may be introduced after 3–4 weeks, only if advised by your surgeon.
  • Avoid rubbing, pressing, or using harsh towels and cleansers on the face during the first weeks.

Sun and Heat Exposure

Newly transferred fat cells are sensitive to heat and UV exposure:

  • Avoid direct sunlight for at least 2–3 weeks.
  • Use a high-SPF sunscreen daily when outdoors.
  • Refrain from solarium, sauna, or steam rooms for the first month.

Nutrition and Hydration

Proper nutrition supports tissue healing and fat cell viability:

  • Consume protein-rich and antioxidant-rich foods (fish, eggs, vegetables, vitamin C).
  • Drink plenty of water to maintain healthy circulation and hydration.
  • Avoid rapid weight loss, which may reduce the longevity of grafted fat.

Follow-Up and Recovery Timeline

The first follow-up is usually within 7–10 days. A portion of the injected fat naturally resorbs; this is expected.
The final contour stabilizes by around the third month. In some cases, a secondary touch-up session may be considered to enhance symmetry or volume retention.

Note: Healing timelines and post-care details may vary depending on injection areas, fat volume, and individual metabolism. Always follow your surgeon’s specific recommendations.
